Assumption of Risk, Waiver, and Release of Liability

By entering and using the facilities at Jolly Jacks LLC, I acknowledge and agree to the following:

1. Assumption of Risk

I understand that participation in activities at Jolly Jacks LLC involves inherent risks, including but not limited to falls, collisions, equipment misuse, and other hazards that may result in injury, illness, permanent disability, or death. I voluntarily assume all risks associated with my participation and/or the participation of any minor child(ren) under my care.

2. Responsibility for Children

I understand that Jolly Jacks LLC is a parent-supervised facility. I agree that I am solely responsible for the supervision, safety, and behavior of any child(ren) in my care at all times. Children must not be left unattended under any circumstances.

3. Compliance with Rules and Policies

I agree to follow all posted rules, guidelines, and staff instructions while on the premises. I understand that Jolly Jacks LLC reserves the right to deny or revoke access to any individual who fails to comply with policies or engages in unsafe or inappropriate behavior.

4. Release of Liability

I hereby release, waive, discharge, and hold harmless Jolly Jacks LLC, its owners, employees, contractors, and affiliates from any and all claims, liabilities, damages, or expenses arising from:

• Personal injury or illness

• Property loss, theft, or damage

• Accidents or incidents occurring on the premises

This release applies to both known and unknown claims.

5. Indemnification

I agree to indemnify and hold harmless Jolly Jacks LLC from any claims or damages resulting from my actions or the actions of any child(ren) under my supervision.

6. Payment for Damages

I agree to be financially responsible for any damage to equipment or property caused by intentional misuse, negligence, or reckless behavior by myself or any child(ren) in my care.

7. Medical Responsibility

I understand that Jolly Jacks LLC does not provide medical insurance. I am solely responsible for any medical expenses incurred as a result of participation in activities at the facility.

8. Dispute Resolution

I agree that any disputes arising from this agreement shall first be attempted to be resolved through good-faith mediation. If further action is required, disputes shall be handled in accordance with the laws of the State of Georgia.

9. General Facility Policies

No Weapons Policy

For the safety of all guests, weapons of any kind (including concealed or open carry) are not permitted on the premises. The only exception is authorized, on-duty law enforcement.

Parent & Guest Guidelines

• Children must be supervised at all times

• No adult entry without a child

• No outside food or beverages (unless approved for parties/events)

• No alcohol permitted

• Jolly Jacks LLC is not responsible for lost, stolen, or damaged personal items

• Guests must follow all posted safety rules and staff instructions

Children’s Conduct

• No hitting, pushing, biting, or rough play

• No inappropriate language

• Respect others and equipment

• Clean up after activities when applicable

Jolly Jacks LLC reserves the right to ask any guest to leave for unsafe or disruptive behavior.
